FOSTER AGREEMENT AMENDMENTS 1) All foster care givers must sign a Volunteer Application, Foster Care Agreement, and the signature form attached to this document (Foster Agreement Amendments) prior to taking in any foster care animals. Any animals taken in before completing the paperwork will not be considered N.A.B.S. animals and N.A.B.S., Inc., will not be responsible for them. 2) We will rescue from local shelters primarily. Please contact Pam Sanidakos when you are available to foster an animal. She will inform you what is available and the history of the animal. We will only take animals from individuals in extreme cases and then only with board approval. If you take an animal(s) from an individual without prior board approval, the animal becomes YOUR responsibility, not N.A.B.S.’ 3) If an animal is taken in to the program and the volunteer decides to take the animal to a veterinarian other than the ones we have an agreement with and without prior approval from a member of the Board, then the volunteer will be responsible for all costs. If a vet visit is approved by the Board and the animal is taken to a veterinarian other than the ones we use, then the volunteer will be responsible for all costs above what we would pay to our contract veterinarians. Some of these non-covered costs are: a) Fecal exams – all animals are wormed with Panacur. b) Shampoo, nail clips, bathing c) Routine vaccinations or prescriptions unless we do not have the Rx on hand. Treatments that are covered (please read completely): a) Heartworm testing (dogs only) – one year or older b) FELIV testing (cats only) – kittens under 6 months of age c) FELIV/FIV testing (cats only) – kittens over 6 months of age d) Trim nails, only if excessively long e) Grooming at groomer if necessary and approved by Pam Sanidakos. If your foster animal becomes sick, please contact Pam S. at 281-330-5238 to get prior approval to take to a veterinarian. 4) VACCINATION AND WORMING SCHEDULE: DOGS AND PUPPIES Worming: Worm with Panacur orally (1 cc per 10 lbs., once daily for 3 consecutive days, skip 14 days, reweigh and repeat for 3 consecutive days) Vaccinations: 1 vaccination (5 in 1) administered by us subcutaneously for adult dogs 1 vaccination (5 in 1) administered by us subcutaneously for puppies at 6 wks, 9 wks, 12 wks. 1 vaccination (Rabies) administered by veterinarian at 12 wks. Heartworm Prevention: Start dog at 5-6 months of age on Heartgard Plus pill monthly or Ivomec mix orally as instructed at 1/10 cc per 4 lbs. Flea Prevention: Use Frontline Plus topically to back of neck (3/10 cc per 10 lbs. once monthly) CATS AND KITTENS: Worming: Same dosing as dogs above. Vaccinations: 1 vaccination (4 in 1) administered by us subcutaneously at 8 wks. 1 vaccination (5 in 1) administered by us subcutaneously at 12 wks. 1 vaccination (5 in 1 booster) administered by us subcutaneously at 16 wks. Heartworm Prevention: Not applicable Flea Prevention: Use Frontline Plus topically to back of neck (3/10 cc per 10 lbs. once monthly) PLEASE SIGN THE ATTACHED SIGNATURE FORM AND GIVE TO PAM SANIDAKOS.
