STERILIZATION AGREEMENT


I, _______________________________________________agree to have this animal, ___________________________________________ (pet name at time of adoption), _______________________________________________________ (pet description),
surgically sterilized within 6-8 weeks after adoption or as soon as a veterinarian doctor recommends this can be done.  I will not hold N.A.B.S., Inc. liable for any excessive medical expenses incurred from this procedure.

I also understand that I will be given the $30.00 reimbursement fee for the procedure and the $10.00 reimbursement fee for the rabies vaccination when I produce documentation that the animal has been spayed/neutered and has had the rabies vaccination.

I also understand that should I not have the animal spayed/neutered with the time frame set forth in this agreement that N.A.B.S., Inc. has the right to reclaim this animal per terms of this document and terms of the adoption contract.
