FORMS

There are four file versions of each form.  If one file format doesn't work, please try one of the others. 

If you can't get some of these files to open, you may need to get a "reader" program.  To get the free Adobe Acrobat Reader for the .pdf version of these forms, click here.  To get the free Microsoft Word Reader for the .doc version of these forms, click here.  The other two formats can usually be opened by products common to most systems.

The preferred method for returning forms is FAXING.  The forms can be faxed back to 1-(610) 300-5238.

Forms that require signatures can be emailed back, but the emailed forms will only be printed and held until it is time to pick up the pet.  Emailed forms will require the original dated signature of the person completing the form before picking up the pet.

All potential adoptive families are prescreened, and receiving the forms in advance will make the adoptions go much more smoothly.  The forms for adoptions are the Pet Adoption Contract, Pet Adoption Application, and the Sterilization Agreement.
